STRESZCZENIE
Zaledwie 17 lat temu określono przyczynę choroby Charcot- Marie-Tooth typu 1A (CMT1A). Po raz pierwszy w historii badań nad chorobami kręgu CMT możliwe było udzielenie porady genetycznej chorym z ujemnym wywiadem rodzinnym. W ciągu 17 lat badań nad podłożem CMT zidentyfikowano setki mutacji w 40 genach a, w niektórych formach CMT poznano patogenezę molekularną. Znajomość patogenezy choroby CMT1A niejako otworzyła drogę do pierwszych terapii eksperymentalnych prowadzonych na modelu zwierzęcym CMT. Największe znaczenie w ujęciu terapeutycznym w chorobie CMT1A miało poznanie efektu dawki genu PMP22 jako kluczowego czynnika patogenetycznego. W chorobie CMT1A prowadzone są obecnie próby terapeutyczne na ludziach z zastosowaniem kwasu askorbinowego. Wyniki badań nad zastosowaniem antagonisty receptora progesteronu (onapriston) na modelu szczurzym CMT1A są również obiecujące.

ABSTRACT
The molecular basis of Charcot-Marie-Tooth disease type 1A (CMT1A) was determined 17 years ago.For a first time in the history of CMT research, genetic counseling was available even for sporadic cases of CMT1A without family history. Within 17 years, hundreds of mutations in the 40 CMT genes have been identified, in some CMT forms molecular pathogenesis was delineated. In the light of molecular pathogenesis first experimental approaches have been applied, on CMT animal models. A most important, known factor determining CMT1A pathogenesis is a gene dosage effect documented for PMP22 gene. In Charcot-Marie-Tooth type 1A disease first clinical trial is conducted with ascorbic acid. The are encouraging results of the onapristone (antagonist of progesterone) experimental therapy on the transgenic CMT1A rats.
